Centre of Innovation for Optimized Aging
Humber River Health is proud to announce the launch of a new Centre of Innovation dedicated to rethinking aging in the digital age. This initiative brings together interdisciplinary experts including engineers, data scientists, nurses, allied staff, physicians and mental health teams to advance care, research, and education focused on supporting people as they age — not just to add years to life, but to add life to those years. By leveraging advanced technologies, data-driven insights, and multidisciplinary collaborations, we are re-imagining what it means to grow older – shifting the paradigm from managing decline to optimizing longevity, independence, and well-being.
With a focus on innovation, equity, and evidence-based approaches, the Centre aims to lead in transforming how aging is experienced and supported across the continuum of care. This includes expanding and enhancing programs already making a measurable difference — such as HEART@Home, which has helped transition over 900 patients and saved over 9,000 Alternate Level of Care (ALC) days since launch; Hospice@Home, which provides seamless 24/7 palliative care and psychosocial support for patients and families; and Remote Monitoring in Long-Term Care, now supporting over 2,200 residents daily across 10 homes, helping to reduce avoidable emergency department visits. Through SCOPE, we’re also connecting primary care providers with hospital resources to deliver more integrated, team-based care and improve access to specialists. Together, these initiatives are creating a more connected, tech-enabled, and compassionate system of care for older adults — wherever they are on their health journey and building new pathways to help people thrive as they age.

Humber River Health has been designed with patient comfort and privacy as a top priority. 80% of inpatient rooms are for single patients, with additional space to accommodate family members. Modern diagnostic equipment enables detailed, accurate patient diagnosis and treatment, and intuitive portals of care ensure easy navigation and reduced walking within the building.
